We understand that the cost of treatment plays a big role in this process. As a starting point, we recommend contacting your insurance carrier to find out what your infertility benefits cover. Once you have this information, our financial coordinators are available to meet with you, either in person or over the phone, to review in detail how your coverage will impact your out-of-pocket costs. Answers To Some Common Questions:
Does TFC require a referral before I can schedule an appointment?
No, we do not require a referral. However, please keep in mind that a referral for a new patient appointment is different than a lab requisition for any male testing. Should you be interested in scheduling an appointment with our andrology laboratory and are not a current patient of TFC, we must have a lab requisition at the time of your appointment before we can accept any specimens being submitted for testing.
What insurance carriers does TFC participate with?
We participate with Blue Cross/Blue Shield, Blue Care Network, Priority Health, Physician’s Health Plan, United Health Care, ASR
Please note: We DO NOT PAR with any government funded insurance plans (i.e. Medicaid, Tricare, etc.). If you currently have one of these plans through one of our participating carriers (i.e. a Blue Cross/Blue Shield Medicaid plan), we would not participate and payment would be due in full at the time of service.
Can I donate sperm here?
No, we do not work directly with sperm donors. While we offer the use of donor sperm to our patients, they order the specimen directly from an approved sperm bank.
Does TFC perform tubal reversals?
