The following information outlines the process for patients under the care of an outside physician/fertility clinic but wish to use The Fertility Center as a testing facility only. Our physicians will not be involved in your plan of care. All treatment decisions/medical advice must come directly from your ordering physician’s office.
Services offered: Ultrasounds and blood work (estrogen & progesterone) are run STAT with same-day results available. Ultrasounds are offered at any of our three Michigan locations (Grand Rapids, Kalamazoo, and Mason).
Ultrasounds/ultrasound guided procedures: available by appointment Monday-Friday with limited availability Saturday mornings.

Blood work: estrogen and progesterone levels are drawn, by appointment, Mon-Sat mornings at our Grand Rapids office. If additional blood work is ordered, our phlebotomists will draw your blood and have it couriered to a Spectrum Health (Grand Rapids) or Bronson laboratory (Kalamazoo). There is a Sparrow laboratory located in the same building as our Mason office, for your convenience.
Monitoring Services Consultation: A consultation with our monitoring services coordinator is required at the start of each treatment cycle, before any monitoring through The Fertility Center. This consultation usually lasts less than 20 minutes and can be done in the office or over the phone. Our monitoring services coordinator will gather information regarding your ordering physician/clinic and treatment plan and review requirements and expectations of the program.
Plan of Care Form: After each consultation, you will need to provide a plan of care form to your ordering physician to complete and return. A new plan of care form must be completed and returned at the beginning of each treatment cycle. This form provides our clinicians with basic health information regarding your type of treatment cycle and medications.
Orders: Your ordering physician must complete and fax an order to our office before your appointment. This order should include the appointment date(s), type of ultrasound, and/or blood work along with your diagnosis code. Standing orders are acceptable. Orders should be faxed attention to our Outside Monitoring Coordinator at 616-988-2010.
Emergency/After-hours care: In the event of a medical emergency. please contact your ordering physician’s office or go directly to your local emergency room. If your ordering physician would like you to be seen, they may contact our office or submit an order for an appointment during regular office hours. Please do not call our emergency/after-hours line.
